Continuity Health Solutions, LLC (providing goods and services under the brand Ally Connected Care  and hereinafter referred to as "Continuity Health Solutions") provides the Web site at  Care.Continuity Health (the "Site"). Continuity Health Solutions is sometimes referred to throughout this Privacy Statement as "we", "our" or "us."

We are committed to protecting your privacy. We value the trust and confidence you place in us.

By visiting the Site or using any of the monitoring and reporting services available on the Site (collectively, the "Service"), you are accepting the practices described in this Privacy Statement and expressly consent to our collection, storage, use and disclosure of personal information about you as described in this Privacy Statement and as may be directed by you on the Site. This Privacy Statement, your use of the Site or the Service, and any dispute related to use of information provided to us, is subject to the then-current version of the Terms of Use available at Terms of Use.



In order to use the Site and our Service, you must logon in accordance with the logon procedures for this Site. If you choose not to logon, you will not be able to access the Site or use our Service.

2. Collection of Personal Information

In providing the Service to you, we will collect the following types of information.

Contact Information

The first time you sign in to the Service, the Service asks you to create an account. To create an account, you must provide personal contact information such as name, e-mail address, mailing information, and country/region.

Billing and Payment Information

For certain services, such as for subscription, health sessions and monitoring services, we will collect credit card or payment account information which we maintain in encrypted form on secure servers. We may also collect financial information such as bank account and tracking numbers.

Personal Health Information

We will also collect health information about you. Here are examples of the types of health information we gather:

Information You Give Us – All personal health information you give us. Examples of the types of information you may provide us include: measurements, such as weight, blood pressure or glucose levels; lab results; medications; health history, and other health or medical information (such as prescription information).

Information Accessed through Third-Party Data Services. When you use our Service, you are may access health-related information about you that is stored with third party-data services such as Microsoft HealthVault or Google Health, and such information will then be available to us.

Third-Party Information – Health-related information about you received from third-parties (such as nurses, doctors or family members).

  • Social Security Number
  • Unique Identifiers, such as name, account numbers and password.
  • Demographic Information, such as age, education, gender, and zip code.


We use cookies with this Service to enable you to sign in and to help personalize the Service. A cookie is a small text file that a Web page server places on your hard disk. You control who can access your personal health information. If you choose to, you can share your information with others. We use information collected through the Service, including personal information and personal health information, to provide the Service and as described in this Privacy Statement. We do not use or disclosure your information except as described in this Privacy Statement or as required by law. We do not sell your personal information to third parties. Information collected through the Service is stored in the United States.

To Protect Us or Third-Parties

We may access and/or disclose your personal information to protect and defend our rights or property (including the enforcement of our agreements) or to act in urgent circumstances to protect the personal safety and welfare of users of the Site or members of the public.

To Provide and Improve Our Services

We use the information that you provide for such purposes as responding to your requests, customizing future browsing for you, improving our Service, and communicating with you. To Track Site Traffic Information

We use non-business IP addresses and other Automated Information collected only to track aggregate information regarding Site traffic. We do not collect any personal information via a user's non-business IP address. One of the uses for which this data is gathered is for statistical purposes so that we may analyze trends and performance over time.

To Comply with Law or for Other Legal Purposes

We may need to disclose certain information to comply with a legal requirement, such as a law, regulation, court order, subpoena, search warrant, in the course of a legal proceeding or in response to a law enforcement agency request. If there is a change of control in our business (whether by merger, sale, or otherwise), or a sale or transfer of its assets, customer information could be sold or transferred as part of that transaction and your personally identifying information potentially used by the purchaser or other recipient of said information, but only as disclosed to you by such purchaser.

For Data Aggregation

We may disclose aggregated, non-personal data generated from the Service to third parties. For example, we might inform third parties regarding the number of users of a certain Health Session program or that "30% of our users live west of the Rocky Mountains." Aggregated, de-identified data may also be used for health trend analysis, disease control and in assessing the effectiveness of various health programs. Depending on the circumstances, we may or may not charge third parties for this aggregated, non-personal data. You grant us the right to accept payment or other remuneration for such data. We require parties with whom we share aggregated, non-personal data to agree that they will not attempt to make this information personally identifiable.

For Use with Third Party Service Providers

We will occasionally hire third party service providers to provide limited services on our behalf, such as customer service companies, billing companies, data integration firms, and other providers as needed to perform the Service. We may give those third party service providers only the personal information they need to deliver the service for which they have been hired. Continuity Health Solutions will require such third party service providers to maintain the confidentiality of any information they receive.

5. Sharing Your Information

A key feature of the Service is the ability to share your health information with people and services who can monitor your health condition and help you reach health-related goals. For example, you can share health information from records you control to: co-manage the health of a family member; use products and services that can improve or monitor your health; consult with your health care provider; or, provide fitness information to coaches and trainers.

You can share your personal health information by sending a sharing invitation e-mail generated by the Service. If the person accepts your sharing invitation and has or creates a Service account, you have given him or her access to your shared information as specifically designated by you. You can specify how long they have access and whether they can modify your information. This shared access to your invitation will also allow invitees to participate in your message center, where you can share additional information about yourself.

You can also share personal information and health information with health care professionals who you select for monitoring services. You decide which health care professionals you want to use and what information they can access. You must authorize any access by a health care professional.

If you grant access to your information to other individuals, Web sites or health care professionals, you understand that these other individuals, Web sites or health care professionals can access your information and store a copy of it. Any copies of your information is governed by that individual's, Web site's or health care professional's privacy policy. Others may be able to view your information from such copies. We are not responsible for the use of your information by others to whom you grant access to your information.

6. Protecting the Confidentiality and Security of Information

We are committed to protecting the security of your information. We maintain physical, electronic and procedural safeguards that are designed to comply with applicable laws and to keep personal information and your health-related data safe. We have adopted commercially reasonable security measures consistent with industry practice that are designed to assist in protecting against the loss, misuse, and alteration of your personal information and health-related data which is under our control. As you probably know, no security system, or system of transmitting data over the Internet, can be guaranteed to be 100% secure and you should not expect, that any information provided to us or to any third party through or in connection with the Site will always remain private. If you have any questions about security on our Site, you can contact us as indicated in Section 2.

7. Account Access, Control and Deletion of Information

You choose whether to create an account with the Service. You control your account and who has access to it. You are responsible for the control of your user names and passwords, and should limit others' access to such information.

You can review and update your account information. You can modify, add, or delete certain personal information, such as your mailing address and email, by accessing your account through the Site. You may also edit certain of your health information. However, if you have shared any of your personal or health information with any third parties, any modifications of your account information by you will not be automatically reflected in any third party copies of your information. If you wish to modify any of your health information held by any third party providers, such as your physicians, you must contact such third parties. We cannot modify such information on your behalf.

You can close your account at any time. If you close your account, we will no longer provide access to your personal information or personal health information, except as required by law. Personal information and personal health information will be retained on Continuity Health Solutions' secure databases as long as required by law.

While we still have your data on our systems, you will be able to receive a list of individuals or entities to whom you have or had granted access or who have otherwise received disclosures of your personal information or personal health information.

8. Links

The Site may include hyperlinks to other sites and third-party advertising. When you link to the site of an unrelated third-party, this Privacy Statement no longer applies. Be aware that we are not responsible for the privacy practices of such other sites. We encourage our users to be aware when they leave our Site or link to outside IP addresses and to read the privacy statements of each and every Web site and IP address that collects personally identifiable information. Continuity Health Solutions makes no representation with regard to the policies or business practices of any sites to which you link from this Site, and is not responsible for any material contained on, or any transactions that occur between you and, any such site. This Privacy Statement applies solely to information collected by this Site and by Continuity Health Solutions.

Some pages may have the look and feel as though they are part of our site, or may be framed by our Site, but are operated by a third party service provider. Although additional privacy statements may be used by the operators of these pages, we have required these third party operators to collect, disclose and, or use your information with the protections and obligations afforded to you under our Privacy Statement. We do not use masking technology to hide which Web site you are actually using.

We are committed to protecting the privacy of children. Neither Continuity Health Solutions nor any of its services are designed or intended to attract children under the age of 13. We do not collect personal information from any person we actually know is under the age of 13. A parent or guardian, however, may use the Site to input personal information and personal health information for a minor. The parent or guardian assumes full responsibility for ensuring that the registration information is kept secure and that the information submitted is accurate. The parent or guardian also assumes full responsibility for the interpretation and use of any information or suggestions provided through Continuity Health Solutions or the Site for the minor.

11. International Users – Applicable Law

We collect, process, store, disclose and dispose of personal information that you submit to the Service in accordance with applicable United States law. Please note that the personal information you submit to the Service will be transferred to the United States to be processed by us in order to provide the Service to you or for such other purposes as set forth in this Privacy Statement. If you are not a resident of the United States, you acknowledge and agree that we may collect and use your personal information, as discussed in this Privacy Statement, outside your resident jurisdiction. In addition, we may store such personal information in the United States, outside your resident jurisdiction.
